Distal ulna is a crucial part of the distal radio-ulnar joint, providing rotation to the forearm. The fractures of the distal ulna need to be fixed properly and on time to ensure proper movement and stability of the wrist. The Ulna Hook Plate provides stable distal ulna fixation with the help of locking screws and distal hooks that shape the ulna styloid.
A forearm is the portion of the arm situated between the wrist and elbow, that is made of two bones that are relatively parallel to each other, namely radius and ulna.
Ulna Hook Plates are used in the following types of fractures in the Ulna, as per the AO guidelines.
Forearm fracture refers to the situation in which the bones in the forearm, i.e., the radius and ulna are either broken or cracked. The break or crack could be anywhere across the forearm – near the wrist, in the middle or towards the end of the bones, that is near the elbow.

It has to be noted that not all the fractures of the distal ulna require internal fixation through surgeries. Simple ulnar styloid fractures may be treated using simple symptomatic treatment also.
Radial head plates are used when there is a severe damage to the radial head, i.e., multi-fragmented fractures.

Pre-surgical:
The surgeon goes through the investigations of the patient, such as X-Ray, MRI and CT/CAT, and counsels the patient and the attendees regarding the procedure and the expected outcomes of the surgery.
Positioning the patient:
The patient is usually made to lie down in supine position, with the arms stretched out completely.
Contouring:
Contouring may not be necessary as the plate is pre-contoured. However, if it is absolutely necessary, contouring may be done using flat-nose pliers.
Screw Insertion for Plate Fixation:
Based on the requirement and the process followed by the surgeon, the screw is drilled inside the plate using drill bits. The plate is fixed distally using appropriate locking screws.
Prophylactic Antibiotics:
To treat the prevailing infections and to avoid the postoperative infections, the doctor prescribes antibiotics prior to the surgery. Administering antibiotics also ensures better healing post the surgery.
Surgical Procedure:
The surgical incision is made toward the ulnar styloid between the flexor carpi ulnaris and extensor carpi ulnaris tendons.
Reduction of the Fracture and temporary plate placement:
After exposing and cleaning the fracture, the pointed hooks of the plate are secured around the tip of the ulnar styloid for reference. In case of simple fractures of the ulnar neck, the plate needs to be fixed at the subcutaneous border of the distal ulna.
Screw Placement Verification:
Fluoroscopic imaging can be used to verify the proper placement of the plate, and other anomalies.
